From 1 July 2022, the Mansfield District Hospital will be the main provider of aged care services.

Community Bus 

A Council initiative to support resident community members who need transport to do shopping, appointments or have a coffee. The bus operates every Thursday (except Public Holidays) and provides pick up and return to your home.

  • Cost - $6.00 round trip
  • Pick up time – First pick up is at 9.30am. Passengers are dropped in Erril Street next to Delatite Dental
  • Return – The Bus leaves Erril Street at 11.30am

Bookings are essential and can be made through Customer Service located in the main Shire Office, or by phone on 03 5775 8555.

Regional Assessment Service

If you need to access basic Government funded in-home support under the Commonwealth Home Support Programme (CHSP), you will need to be assessed by the Regional Assessment Service (RAS). Assessment for Home Care Packages takes place by the Aged Care Assessment Service (ACAS) which operates from Gateway Health following a referral from the RAS officer. Vulnerable Persons Register 

Council provide support and monitoring for the vulnerable, aged and disabled community members through the vulnerable persons register and health promotional activities. Please contact the Community Connections Officer, Tracey Dalziel on (03) 5775 8566

Community Connections Officer

The senior community are being supported by providing the Community Connections Officer until September 2023. This role provides community support and engagement, assists with social connectedness and service access and helps Council monitor the availability and access to services and identify gaps that require advocacy and planning. My Aged Care

If you are 65 years and over and just beginning your aged care journey, this is your first step. You can see what services are available to help you stay in your own home, or what to expect in an aged care home. You can also find out if you are eligible and how much it might cost.

Commonwealth Home Support Program (CHSP), and the Home and Community Care (HACC) Program for Younger People (PYP)

Mansfield District Hospital is the main provider of CHSP and HACC-PYP services in Mansfield Shire through their Support@Home program

Services that can be provided include:

  • Personal Care
  • Domestic Assistance
  • Home safety and maintenance
  • Transport services provided by volunteers
  • Meals on Wheels
  • Flexible Respite
  • Social Support Groups
